Each fall, all childcare facilities licensed by the State of Tennessee are required to provide information regarding the flu vaccine to our families. As proof that we provided this information, we are also required to collect signatures from each family. Please note, signing this form is not an agreement to get a flu vaccine, rather it is acceptance that we've provided this information.
